Full Cycle Digital Engineering For A Remote Patient Monitoring Platform Built Across 3 Growth Stages

Industry
Healthcare & Life Sciences
Geography
Denmark, Europe
Cooperation Period
6 months

About the Client

The client is a Nordic founded healthtech company specialising in Remote Patient Monitoring (RPM) and chronic disease management. At the point of engagement, the company was early stage, strong clinical domain knowledge, an ambitious product vision, but no digital presence, no mobile platform, and no pipeline of hospital relationships.

The operating environment imposed hard constraints from the start. GDPR governed all patient data handling. The EU Medical Device Regulation (MDR) applied to any software classified as a clinical tool. ISO 13485 set the quality management standard for device connected systems. Every workstream, from the first marketing page to the final EMR integration, had to clear these thresholds.

By 2023, the company had been acquired by a leading European virtual care provider, with the combined platform serving hospitals, clinics, and national health agencies across 12+ countries.

Challenge

The client came to Clixlogix at a point where many healthtech startups fail -the gap between having a viable clinical concept and having the digital infrastructure to prove it to hospital procurement teams. The challenge was not a single build. It was a multi-year progression through three lifecycle stages, each with different constraints, different buyers, and different technical requirements.

Stage 1: Startup (2016 to 2017)

No website, no lead pipeline, no CRM. The client needed to become discoverable by hospital decision makers across Nordic, DACH, and Benelux regions while meeting EU healthcare communication and data handling standards from the first form submission.

Stage 2: Growth (2018 to 2020)

Early pilots were running, inbound interest was building, but the marketing infrastructure could not scale. Content existed in two languages; EU tenders required eight. Conversion tracking was rudimentary. The client had no visibility into which channels, regions, or buyer personas were driving qualified leads. The underlying technology needed to start preparing for platform-grade integrations.

Stage 3: Scale and Exit (2020 to 2023)

After getting funding, the business case shifted from market validation to platform delivery. The client needed production grade mobile apps with medical device connectivity, FHIR-compliant APIs for interoperability with hospital EMR systems (including Cerner Millennium across the EU network), predictive analytics, and automated compliance infrastructure capable of passing MDR and GDPR audits in 12+ regulatory jurisdictions.

Solution

Clixlogix operated as the client’s primary digital and engineering partner across all three lifecycle stages. The engagement started with two people on a WordPress build and grew into a cross-functional team spanning marketing, frontend, backend, mobile, DevOps, and compliance engineering. At peak, six workstreams ran concurrently.

Stage 1: Startup: Digital Foundation and Market Entry (2016 to 2017)

Three workstreams ran in parallel: website development, digital marketing, and CRM integration. The objective was a procurement ready digital presence within six months.

Website Development

  • Built a responsive, multilingual WordPress site structured around how hospital procurement teams evaluate vendors: clinical capabilities, compliance credentials, integration readiness, and deployment evidence.
  • Configured language specific subdirectories for Nordic, DACH, and Benelux audiences with localised navigation and region-appropriate imagery.
  • Implemented GDPR compliant lead capture forms with conditional routing by inquiry type. All submissions fed into Zoho CRM with automated lead scoring.

Digital Marketing

  • Conducted healthcare specific keyword research across four languages targeting clinical procurement terminology. Implemented multilingual on page optimisation with hreflang attributes, translated metadata, and MedicalOrganization schema markup.
  • Executed technical performance work, server side rendering, image compression, lazy loading, Core Web Vitals optimisation. Built an internal linking architecture across service, compliance, and clinical use case pages.
  • Developed a compliance first content strategy aligned with EU healthcare communication guidelines, covering MDR claims substantiation and GDPR transparency requirements.

Zoho CRM Integration

  • Configured Zoho CRM with custom modules mapping the healthcare procurement cycle, inquiry, clinical evaluation, pilot approval, contract negotiation.
  • Built automated lead routing by geography, inquiry type, and deal size. Webhook integration with the website eliminated manual data entry.

Stage 1 Workstream Summary:

Workstream Deliverables Outcome
Website Development Multilingual WordPress site, GDPR forms, conditional routing Procurement-ready presence across 3 EU regions
Digital Marketing 4-language keyword strategy, technical optimisation, compliance content 78% organic traffic growth in Year 1; first hospital inquiry in 6 months
Zoho CRM Custom procurement modules, automated routing, webhook integration Structured lead pipeline from day one

Stage 2: Growth, Scaling the Marketing Engine (2018 to 2020)

Four workstreams ran through this stage: CMS expansion, digital marketing and analytics, content marketing, and early platform integrations via Zoho Creator.

CMS Architecture Expansion

  • Scaled the WordPress CMS from two languages to eight for EU tender compliance. Built translation workflows with staging environments per locale, editorial approval chains, and synchronised publishing.
  • Implemented taxonomy and tagging by clinical specialty, compliance standard, and deployment region for procurement evaluator navigation.

Digital Marketing and Analytics

  • Built a custom analytics dashboard in Google Data Studio integrating search performance, session data, and CRM lead records, segmented by country, language, and clinical specialty.
  • Created a conversion attribution layer linking organic sessions to CRM leads for cost per lead calculations by region and content asset. Automated weekly reporting flagged ranking shifts and content decay.
  • Ran systematic conversion optimisation, A/B tested forms, CTAs, and social proof elements. Implemented behavioural triggers (exit-intent, scroll-depth CTAs, personalised CRM sequences). Improved content-to-lead conversion by 2.3x across two buyer personas.

Content Marketing

  • Produced targeted campaigns for clinical decision makers, RPM reimbursement white papers, clinical workflow guides, interoperability checklists for hospital IT teams.
  • Published anonymised pilot evidence demonstrating patient engagement metrics and clinician adoption rates.

Zoho Creator Integrations

  • Built custom Zoho Creator applications for clinician onboarding, device assignment tracking, and pilot milestone reporting.
  • Developed a pilot management dashboard with cross country visibility into enrolment, device activation, and clinician feedback.

Stage 2 Workstream Summary:

Workstream Deliverables Outcome
CMS Expansion 8-language CMS, translation workflow, taxonomy system Tender-ready content across all target EU markets
Digital Marketing & Analytics Dashboard, attribution, CRO program 2.3x conversion lift; top-5 for 15+ keywords in 3 regions
Content Marketing Persona campaigns, white papers, pilot evidence Steady lead flow from hospitals in 5 EU countries
Zoho Creator Onboarding apps, pilot dashboard Multi country pilot management infrastructure

Stage 3: Scale and Exit: Platform Engineering (2020 to 2023)

Six workstreams operated concurrently, backend architecture, mobile apps, EMR integration, analytics, compliance engineering, and ongoing digital marketing.

Backend Platform Architecture

  • Microservices backend on Node.js and PostgreSQL, orchestrated via AWS Fargate. Independent scaling for data ingestion, API gateway, notification, and analytics services.
  • FHIR-compliant REST APIs for bidirectional data exchange, patient demographics (FHIR Patient), clinical observations (FHIR Observation), care plans (FHIR CarePlan), device telemetry (FHIR Device).
  • Multi tenant data architecture with row level security. API rate limiting for burst BLE device sync traffic during peak clinical hours.

Cross-Platform Mobile Apps (iOS and Android)

  • React Native apps with role based UI rendering for patient and clinician interfaces from a shared codebase.
  • BLE integration with certified medical devices (blood pressure cuffs, glucometers, pulse oximeters, ECG monitors). Dedicated service profiles, custom pairing flows, and data normalisation to FHIR Observation resources per device type.
  • Offline first data capture (local SQLite, automatic background sync, chronological upload with conflict resolution). WebRTC encrypted teleconsultation with scheduling, waiting room, session recording, and audio fallback.
  • Push notification infrastructure, medication adherence, appointments, missed measurements, clinical anomaly escalations.

EMR Integration (Cerner Millennium)

  • Integration adapters using Cerner Millennium Open APIs and HL7 FHIR R4 endpoints. Vitals flowed from BLE devices through FHIR APIs into Cerner clinical documentation; care plan updates synchronised back to mobile apps.
  • Reconciliation engine for data conflicts between platform observations and clinician edited Cerner records. OAuth 2.0 SSO bridges for frictionless clinician access across both systems.
  • Staged rollouts: sandbox integration testing, data integrity validation, latency benchmarking, edge case handling (device timeouts, partial sync, duplicate records).

Additional EHR system integrations were delivered as addon adapters within the same FHIR integration framework. Cerner Millennium was the primary integration; subsequent hospital-specific EHR connectors extended coverage across the client’s full network to 30+ systems.

Analytics and Predictive Intelligence

  • Predictive patient risk scoring (AWS QuickSight): vitals trends, medication adherence, historical episodes. Clinician workload dashboards for capacity planning.
  • Population health views for public health agency clients, deidentified outcomes by condition, region, and care pathway.

Compliance and Audit Engineering

  • Automated MDR/GDPR audit logging: every data access, workflow action, and configuration change recorded with identity, timestamp, action type, and scope.
  • DSAR tooling for GDPR Article 15/17 responses. RBAC mapping clinical roles with API-level data visibility enforcement. ISO 13485 documentation for change control, incident management, and post-market surveillance.

Digital Marketing (Parallel Workstream)

  • Continued marketing operations throughout the platform build. Published technical content for clinical IT evaluators: architecture overviews, FHIR compliance documentation, security summaries.
  • Maintained analytics infrastructure tracking marketing performance alongside platform adoption metrics.

Stage 3 Workstream Summary:
